Home Blood Pressure Telemonitoring Linked with Community Health Workers to Improve Blood Pressure
Led by Dr. Yvonne Commodore-Mensah from Johns Hopkins University, the LINKED-BP team aims to recruit patients from Federally Qualified Health Centers (FQHCs) in Maryland and DC to test whether home blood pressure monitoring and lifestyle coaching can lower blood pressure.
About LINKED-BP
The LINKED-BP study collaborates with community health centers and primary care practices serving underserved populations in Maryland and DC. The study targets patients with elevated blood pressure or untreated stage 1 hypertension. Participants are either provided with a home blood pressure monitor and access to a community health worker and a mobile health app, or just the monitor. Over 12 months, the team will assess if the comprehensive intervention leads to better blood pressure control compared to the monitor-only group, aiming to enhance hypertension management in these communities.

LINKED-BP Community Partners
The LINKED-BP Program fosters a collaborative relationship with its community health partners, including Johns Hopkins Community Physicians (JHCP) and Choptank Community Health System, to implement a comprehensive, patient-centered, multi-level intervention for blood pressure management. Through these partnerships, the LINKED-BP Program provides standardized training to primary care practices within these health systems, focusing on office-based blood pressure measurement protocols and patient empowerment strategies for self-managing blood pressure. Practice champions, identified from each participating site, play a pivotal role in facilitating seamless communication, recruitment, and implementation of the LINKED-BP Program. These champions work closely with the study team to ensure that the intervention is tailored to the unique needs and workflows of each practice, thereby enhancing its integration and sustainability. Furthermore, the collaboration involves regular feedback mechanisms, quarterly progress updates, and joint problem-solving to address any challenges that arise during the implementation process. By leveraging the strengths and resources of its community health partners, the LINKED-BP Program aims to enhance the quality of care, promote health equity, and improve blood pressure control among underserved populations in Maryland and the District of Columbia.
